FOR MEDICARE PATIENTS:
ROCKY MTN HEALTH MEDICARE ADVANTAGE PLANS:  

For those of you who are losing your Rocky Mountain Medicare Cost Plus Health Plan (RMHP) or other Medicare patients, Sage Health Care will be in network for the new RMHP Advantage Plan and will be able to provide services under this plan as of 1/1/2021. These HMO plans offer $0 or $39 Monthly Premium Plans (they replace your original Medicare Plan). Sage is getting some requests by various Insurance Companies for your medical records.That tells us you are trying to sign up for a non-ACA Health Care Plan. The reason these companies are requesting records is because they are looking for “pre-existing conditions” to deny or limit coverage for you. We are really strong supporters of the Affordable Care Act (ACA or Obama Care) health plans. Many of these non-ACA plans we are not contracted with, so you may be charged as a self pay patient.  (A superbill will be provided for you to submit to your insurance. We strongly recommend you consider ACA plans such as those provided through the SW Alliance.
