Our services include “Live Laugh! Leak?”, our unique program for Pelvic Floor Rehabilitation; Primary Care for women including evaluation and management of common health problems; women’s wellness/annual exams; management of menopause and hormone therapies; birth control; health education; tests and referrals to specialists as needed.

Meet Us

Here at Sage Health Care we are proud not only to provide comprehensive and professional health care services to our patients, but to do this in a uniquely warm, comfortable and personable setting.

We are focused on each individual woman during her visit; we schedule visits lasting 1 hour for women’s wellness / annual exams or pelvic floor strengthening sessions as opposed to the usual 20 – 30 minute visit offered by many other clinics.

Our nurse practitioners provide direct care to you throughout your visit. Providing prompt and dependable follow up is one of our priorities.

Our primary goal is to meet the needs of our patients.

Debbie schedules regular appointments Monday and Thursday (https://square.site/book/A11P57Y4SDMD6/in-clinic-at-sage-health-care-durango-co) , and is available for Telemed visits Tuesday, Wednesday and Friday (https://square.site/book/X4X8QQ770RCPQ/telemedicine-at-sage-health-care) .

Debbie Meyers, Nurse Practitioner is excited to announce her first anniversary as the owner of Sage Health Care, LLC. Debbie is an Adult and Pediatric Nurse Practitioner who has been practicing as a women’s health care nurse practitioner for over 20 years.

Deb started her 35-year nursing career working with the geriatric population in long-term care, home health, and hospice care, receiving the Colorado Health Care Association Nurse of the Year Award in 1983. Her journey as a Nurse Practitioner took an interesting turn and led her into working with college age and later school age populations and eventually into the area of women’s health.

She has worked at both the individual and community level. Now, she continues her practice in women’s health at Sage Health Care. She also stays active with her cherished hospice nursing, conducting patient “face to face” visits for Montezuma Hospice in Cortez, CO.

Debbie began her nursing education with an Associate Degree in the Arts and Sciences and in Nursing. She received her BSN at the University of the State of New York Regents program. Following this, she was certified as a Pediatric and Adult Nurse Practitioner at the University of Colorado. With the help of a “Friends of Nursing” Greta Pollard Scholarship and a Colorado Nurses Association Virginia Paulsen Rural Nursing Scholarship, Deb received her Master’s degree in Community Health Nursing at the University of Colorado Health Sciences Center in Denver in 1993, where she also received her Nurse Practitioner education in Adult -Geriatrics and Pediatrics.

Deb is certified as an American Nurses Association (ANCC) Adult and Pediatric Nurse Practitioner (now seeing patients ages 15 and up) providing women’s primary health care. Deb is a member of the American/Colorado Nurses Association and is currently active in the Health Coalition of SW Colorado (focused on prevention, early detection and support of individuals with cancer).

Debbie’s philosophy is to “care” for and be a partner in health with each woman, taking the time to listen with empathy and to mutually develop practical approaches to promote balanced health and wellness in all stages of life. Her wide range of nursing practice, life experiences and temperament contribute to her compassionate, yet pragmatic and humorous approach to health care.

Her services include women’s wellness exams /pap smears, sports physicals, diagnosis and treatment of common health problems, health education, contraception, management and care of menopause/hormone replacement, cryo treatment of skin lesions,  interpretation of labs and referrals to specialists as needed.

Maureen Goldman, CNP, BSN, BCB-PMD, Women’s Health Nurse Practitioner

Maureen Goldman, CNP, BSN, BCB-PMD

Maureen “Mo”Goldman has been a practicing nurse for 40 years. Her diverse experience includes Medical-Surgical nursing, Obstetrics, Labor and Delivery, Pediatrics, Long Term Care, School Nursing, Public Health and working with developmentally delayed patients. She found her home as a Women’s Health Nurse Practitioner working in a Primary Care Setting, experiencing the rewards of helping to empower women in self-care and self-advocacy. In recent years Mo has focused on women’s pelvic floor dysfunction issues which can be positively resolved with therapies not requiring medications or surgery.

Mo provides treatment for a range of issues including bowel and bladder incontinence, pelvic organ prolapse, voiding dysfunction, post-surgical pain, rectal and pelvic pain in women. She also incorporates other modalities in her treatment such as Chi Nei Tsang, Reiki, Myofascial Release, and Cranio-Sacral work. She is bilingual and enjoys working with Spanish speaking patients.

Mo received her B.S. in Nursing at Texas Women’s University, and her Women’s Health Care Nurse Practitioner certification at University of Texas Southwestern Medical School in Dallas, Texas. Mo also achieved Board Certification by the Biofeedback Certification International Alliance (BCIA ) in Pelvic Floor Muscle Dysfunction (PFMD) making her the only clinical provider in SW Colorado and the Four Corners region to achieve this highly specialized certification.

Mo and her husband Charlie live in Bayfield, Co and enjoy spending time both indoors and outdoors with their children and grandchildren. Helplessly addicted to, and life-long supporters of, organic gardening, they have donated land for Community Supported Agriculture in Albuquerque, NM, where they support Biodynamic Farming practices. Mo also enjoys knitting, weaving and continuing her studies in health and wellness for herself and her clients.

Amy has 4 married children and 8 amazing grandchildren from 4 to 25 years old.
